The disclosure log shows requests that the Trust has received for information under the Freedom of Information Act 2000 and responses we have provided. The selection of what we publish is made by considering if we believe that there may be a wider public interest, when information is frequently requested, or when information is not already available elsewhere. Therefore all reference numbers will not be sequential. Personal details such as names and addresses (both postal and email) are redacted to protect requesters’ privacy.
Our disclosure log is anonymised but organised by folder structures of year and months with brief description of request. Please note the month folder is based on the date the request was received.
